School forms

The Admissions Application should be submitted to show interest in the school, after you have toured or attended an open house and when you are ready to either be placed directly in a class or join the waitlist for a class.  The Application for Admission should also include a $60 application fee, unless you are joining a waitlist, in which case the application fee would only be due if a spot became available.

APPLICATION FOR ADMISSION

After you have submitted the application for admission and been assured a spot with our school, an enrollment contract will be sent to you. You are required to return this contract, signed and with a deposit of one month’s tuition, by May 1st. This deposit is applied toward May’s tuition of the upcoming school year. This deposit will lock in your child’s spot. Eight tuition payments begin on August 1st and continue with the last payment due on April 1st (there is no payment due in January).

The following are the forms we need to have on file by the first day of school.  These can be mailed to the school over the summer or dropped off during open hours. Copies are always available from the school.  The following forms should only be submitted by families who have been assured a spot in the school.  The following forms are not required to be on the waitlist or to save a spot initially in a class, which only requires the Application for Admission.

Please retain a copy of all forms for your records, in case they become lost in the mail.
