Forms and Documents

New and returning families are required to submit all forms to complete their child's file prior to the first day of school, including online forms below. 

All student files require:

• Emergency Form 
• Healthy Inventory Form (Part 1 & 2) 
  • Part 1- Completed by Parent/Guardian
  • Part 2- Completed by health care provider
• Blood Lead Testing- Completed by health care provider
• Immunizations - Form MDH 896 (completed by health care provider) OR computer generated immunization record
• Magothy Permissions Form (link)
• Magothy Helping Grown Up Form (link)
• Getting to Know Your Child Form (link)
• If your child requires medication the Medication Administration Form must be completed by your child's health care provider.
