New and returning families are required to submit all forms to complete their child's file prior to the first day of school, including online forms below.
All student files require:
- Emergency Form
- Healthy Inventory Form (Part 1 & 2)
- Part 1- Completed by Parent/Guardian
- Part 2- Completed by health care provider
- Blood Lead Testing- Completed by health care provider
- Immunizations - Form MDH 896 (completed by health care provider) OR computer generated immunization record

- If your child requires medication the Medication Administration Form must be completed by your child's health care provider.
